Format: choose CSV, choose Comma, and enable Add Header Line
Encoding: choose Unicode (UTF-8)
Filter: don’t change anything
Modifications: don’t change anything

To Transfer Contacts from Computer to iPhone via iCloud:
You have contacts saved as VCF format on computer and want to transfer it to your iPhone. It needs two steps to make it: first import the VCF contacts to iCloud on computer and then turn on iCloud sync on iPhone. See the steps below to see how to transfer VCF contacts from PC to iPhone.
On your PC: Go to iCloud.com and log in with your Apple ID and password > Click the Contacts icon > Click the Gear icon and choose Import vCard... to select the VCF files.

Please be aware that your contacts should be saved as vCard files in VCF format on computer. Besides, set up your email using the stock Mail app or any other third-party mail clients.
On your PC: Open a browser and log in your email account > Attach the VCF file to a new email and sent it to yourself.
On your iPhone: OpenMail app to check the email > Tap to open the VCF file > Then you will get the option to add contacts from mail to the Contacts app. Choose Create New Contact or Add to Existing Contact according to your needs.
